ALLIANZ TRAVEL foreign travel insurance
The Insurer is AWP P&C S.A. Oddział w Polsce (Allianz Travel). The policy name is: LUX MED Group Key Clients The insurance coverage has no age limit. The maximum period of insurance coverage during a one-off stay abroad is 180 days. The insurance covers all countries of the world, excluding Poland and the Insured Party’s country of residence. Reimbursement of treatment costs in the event of an unexpected war or act of terrorism. Reimbursement of treatment costs associated with unexpected treatment of COVID-19 (Note! The insurance does not cover the costs of quarantine).

Enrolment for the insurance
In order to enrol for the insurance, you must be an eligible person declared for the purpose of a medical package that includes foreign travel insurance. Enrolment for the insurance is voluntary. Coverage is granted subject to signing up via the application (in exceptional cases, it is permitted to submit the enrolment declaration on a form):
- individually by each Eligible person, or
- for a group, by a person authorised by the Employer, on a Group insurance enrolment declaration form.

- or, if you cannot sign up for the insurance via the application, by post at: AWP P&C S.A. Oddział w Polsce Dział Sprzedaży Ubezpieczeń Turystycznych, ul. Konstruktorska 12, 02-673 Warszawa, with a note: ‘LUX MED – Aktywacja ubezpieczenia kosztów leczenia w podróży zagranicznej’ (LUX MED – Activation of insurance covering the costs of treatment during foreign travel).
Note: Sending the declaration does not result in the commencement of insurance coverage.
Commencement of the insurance coverage
The starting date of the insurance coverage depends on the date of receipt of the AWP enrolment declaration. The assumed date of receipt of the declaration shall be:
- the date of signing up for the insurance;
- the date of receipt of the letter at AWP – Dział Sprzedaży Ubezpieczeń Turystycznych (Travel Insurance Sales Department)
Expiry of the insurance
The insurance shall remain valid as long as the eligible person holds a medical package that includes foreign travel insurance. The insurance expires automatically:
- on the date of termination or expiration of the agreement concluded between the Employer and LUX MED;
- on the date the person is removed from the list of eligible persons entitled to receive medical care;
- on the day the Eligible person changes the product to a different one that does not include foreign travel insurance.

To verify the insurance coverage, you will need to provide the following data:
- name and surname,
- personal ID No (PESEL),
- date of birth (for persons without a personal ID No, i.e. children up to 6 months of age and foreign nationals who do not have a personal ID No assigned),
- name of the Employer.
To obtain foreign travel insurance coverage, you need to enrol for the insurance and add all your family members who are declared for the purposes of the medical package you hold. You can sign up for the insurance by following the steps below:
