Hospital Care Coordination includes, but is not limited to: organisation of hospitalisation, ongoing telephone contact, continuity of care for procedures, hospitalisation and recovery, and coordination for planned births.
Scope of coordination
The Coordinator facilitates the use of hospital care for you at any stage of your treatment.
- Supports the arrangement of examinations and hospital admission.
- Contacts the loved ones you indicate, providing them with necessary information.
- If necessary, arranges for the relatives' stay in hospital.
- Prepares documentation, collects all post-operative recommendations, referrals and prescriptions.
- Schedules a follow-up appointment.
- Arranges hospital discharge and, if needed, also transportation.
