COVID-19 vaccinations at workplace

As announced by the Council of Ministers, registration is scheduled to open on 4 May 2021 for employers interested in carrying out COVID-19 vaccination campaigns at workplaces. Here's how to go about submitting a request to undertake vaccination cooperation at workplaces:

The employer notifies LUX MED of its wish to start cooperation in the field of Workplace Vaccination by filling in a questionnaire: https://luxmed.limequery.com/126448?lang=pl.

LUX MED verifies the application for compliance with the programme guidelines: https://www.gov.pl/web/szczepimysie/szczepienia-zakladow-pracy and confirms readiness to provide the service.


After receiving confirmation from LUX MED, the employer reports via a form provided on the Government Security Centre website https://rcb.gov.pl. As announced, the form will be available from 4 May. The number of employees willing to vaccinate without providing their personal data should be indicated. The workplace may issue a maximum of one application at the same time, which should meet the following criteria: contain the data of the cooperating Entity Performing Medical Activity (PWDL) – the workplace must obtain its consent, number of reported persons: min. 300, no upper limit (in the case of large applications, more than 10,000 people, the possible staging of vaccine deliveries should be taken into account), the employer, in cooperation with the reported PWDL, should be able to vaccinate all reported persons within a maximum of 5 days ( the number of reported persons and medical entities should be appropriately selected).


The Employer sends to szczepienia_covid@luxmed.pl confirmation of the declaration of LUX MED as an Entity Performing Medical Activity (PWDL) with the Government Security Centre.


A vaccination agreement is signed between the Employer and LUX MED.


Vaccination dates at the workplace depend on the availability of vaccines. The order will be determined based on the correct completion of the form by the employer and the availability of vaccines. Vaccination start dates at a given facility will be indicated by the Government's Strategic Reserve Agency, through an increased offer in the Vaccine Distribution System (SDS) for the serving PWDL.
